Our Office Policies:

  • We ask that you cancel appointment more than 24 hours prior to the appointment will be considered a “No Show.”
  • Patients must come in for a follow-up appointment to have prescriptions refilled. Medications will not be filled over the phone.
  • All test results are discussed at the follow-up appointment. Patients will be notified and asked to come in earlier than the scheduled follow-up appointment if tests results are abnormal.
  • Appointment Cancellations:
    As a courtesy, we attempt to contact our patients to remind of an appointment. We request a call 24 hours prior to cancel or reschedule an appointment to avoid cancellation fees.
  • Payment Policy:
    Payment and/or co-pays are collected at the time of service. Our practice accepts cash, check or Visa/MasterCard and Discover. We file claims for participation insurances and Medicare. The patient is responsible for any unmet deductible or co-insurance.
  • Prescriptions and Refills:
    Refills will only be given at your follow up appointments. If you miss your appointment for an extenuating circumstance, it will be up to the doctors discretion to call in enough medication until your rescheduled appointment.
  • After Hours:
    After hours, there is a telephone recorded message which instructs how to contact the physician on call if an urgent problem arises. In the event of an emergency, please call 911.
